Allied Community Services provides Targeted Case Management (TCM) to children and youth with behavioral and mental health needs—especially those navigating school challenges, multiple providers, or system involvement—and help families coordinate services and stabilize care. Our case managers work directly with families to understand each child’s unique strengths and challenges and help coordinate the services needed for stability, growth, and well-being. Services are individualized and built around the needs of each child and family.
What We Do
An Allied Community Services Targeted Case Manager partners with families to help organize and coordinate services across multiple systems—including schools, providers, and community resources—so families receive the right support at the right time. Support is provided to children and youth within their natural environments such as at home, in school, or in the community, and may include:
- Assessing the strengths, needs, and goals of the child and family
- Developing an individualized service plan to address those needs
- Connecting families to mental health, medical, educational, and community resources
- Coordinating services between providers, schools, and support systems
- Helping families navigate MaineCare and community services
- Attending meetings and advocating for the child’s needs (such as IEP or school meetings)
- Monitoring progress and adjusting services as needs change
- Supporting families in accessing appointments, programs, and community supports
Our goal is to help families build strong support systems and ensure children receive the services they need to thrive.
Who is eligible?
Targeted Case Management services may be available for:
- Children and youth ages 0–20
- Individuals who are covered by MaineCare or the Katie Beckett waiver
- Youth with behavioral health needs, developmental disabilities, intellectual disabilities, or significant medical needs
Referrals may be made by parents, guardians, providers, schools, or other professionals with family consent.
When to Refer
Refer a child or youth if coordination across systems is needed, or if they are experiencing:
- Behavioral or emotional challenges at home, school, or in the community
- School-related concerns (truancy, suspensions, IEP/504 needs, school refusal)
- Involvement with multiple providers without clear coordination
- Family overwhelm navigating services or systems DHHS or child protective involvement
- Frequent crises (ER visits, law enforcement involvement, instability)
- Housing instability or homelessness
- Risk of placement disruption or higher level of care
