Home and Community Based Treatment
A home or community based behavioral health service for youth and families in need of a more intensive treatment than our outpatient therapy. This in-home treatment offers strategies to help children and families manage mental health symptoms, function better in the home, school, and community settings with the overall goal to prevent psychiatric inpatient hospitalization. How Do I Start?
Case managers or parents/legal guardians should complete two forms and fax it to (866) 325-4752. Download the below forms that have been pre-populated with our agency’s information:
Rehabilitative and Community Support Services
Our program provides an opportunity for children and youth ages 0-20 years with cognitive impairments and functional limitations to learn daily living skills and develop/maintain developmentally appropriate skills in the home and the community. This will include learning social skills and creating environments to assist in implementing treatment goals, allowing children to succeed in home, school, and the community to live their most independent life.
Targeted Case Management
Our TCM services help coordinate and identify supports for individuals ages 0-20 years who have mental health needs. Our case managers provide coordination from educational, legal, vocational, and financial aspects, which is determined through a needs assessment. They help to identify and locate treatment services to meet the need of the child and to help parents navigate the mental health system.
Outpatient Therapy
Our agency provides professional assessment, counseling, and therapeutic services to individuals of all ages. We aim to improve functioning, address any symptoms, and improve the overall well-being of our clients by relieving excess stress and promoting positive orientation and growth to facilitate increased integrated and independent levels of functioning.
Community Integration Support Services
Each client in our Community Integration Support Services program works closely with the Community Integration Specialist to develop a plan to work towards achieving a fulfilling and meaningful life from the client’s perspective. The Community Integration Specialist will assist in identifying and facilitating access to medical, social, educational, vocational, and other needs.
