Allied Community Services provides Targeted Case Management (TCM) for children, youth, and families who need support navigating behavioral health, developmental, medical, or social service systems.
Our case managers work directly with families to understand each child’s unique strengths and challenges and help coordinate the services needed for stability, growth, and well-being. Targeted Case Management focuses on advocacy, service coordination, and connection to resources so children can succeed at home, in school, and in their communities.
Services are individualized and built around the needs of each child and family.
Who is eligible?
Targeted Case Management services may be available for:
- Children and youth ages 0–20
- Individuals who are covered by MaineCare or the Katie Beckett waiver
- Youth with behavioral health needs, developmental disabilities, intellectual disabilities, or significant medical needs
Referrals may be made by parents, guardians, providers, schools, or other professionals with family consent.

What is Targeted Case Management?
A Targeted Case Manager partners with families to help organize and coordinate services across multiple systems. Support is provided to children and youth within their natural environments such as at home, in school, or in the community, and may include:
- Assessing the strengths, needs, and goals of the child and family
- Developing an individualized service plan to address those needs
- Connecting families to mental health, medical, educational, and community resources
- Coordinating services between providers, schools, and support systems
- Helping families navigate MaineCare and community services
- Attending meetings and advocating for the child’s needs (such as IEP or school meetings)
- Monitoring progress and adjusting services as needs change
- Supporting families in accessing appointments, programs, and community supports
